Description

Clinical staff to provide family and individual therapy to children and families in their homes.  Also will provide intensive case management services. The children, who have a variety of behavioral and emotional issues, range in age from 3 to 21 years.  An agency supplied three year training program allows Bachelor level clinicians to meet requirements for Mental Health Professional upon completion.   Competitive salary, excellent benefits, supportive work environment.  Full time position, working hours are Monday, Tuesday, Wednesday and Thursday 9 AM to 7 PM. 

Requirements Bachelor’s degree, driver’s license and clinical experience with children and families is required.  Fluency in Spanish is a plus.

Brief Description of Duties:

  • Partner with consumers in achieving self-advocacy
  • Develop WRAP/Crisis Plans with consumers upon request
  • Keep appropriate records and documentation of engagement and provide reports regularly

Requirements:

High School diploma/GED is required.

Current certification in Peer Specialist Services needed.

Associates, Bachelors, or Masters a plus.

Previous or Current participation in the MH system as a recipient of service.

Staff Requirements:

The following requirement shall be met by supervisors of Recovery Coaching/Blended Case Management Services:

  • A Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience.

Other Requirements:

  • Valid driver’s license and good driving record

Job Duties:

  • Supervise up to nine Recovery Coaching staff
  • Meet performance indicators determined by Montgomery County Behavioral Health
  • Responsible for staff compliance with State, County, and agency policies and regulations.
  • Determine eligibility of referrals made for Recovery Coaching.
  • Work closely with the department’s Director of the Case Management/Recovery Coach department at Central.

Job Activities will include the following:

  • Monitoring staff charts, notes, and production
  • Complete regular performance evaluations of staff
  • Provide weekly supervision to staff
  • Coordinate clinical supervision of staff
  • Attend weekly department supervisor’s meeting.
  • Attend required trainings
  • Ability to communicate well with consumers, families, agency staff and referral sources.
  • Have the ability and knowledge to work collaboratively with Montgomery County Behavioral Health staff, Magellan Behavioral Health staff, agency personnel and various outside organizations.

POSITION QUALIFICATIONS:

Bachelor’s degree with major coursework in sociology, social work, psychology, criminal justice, counseling or a related field. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, criminal justice, counseling or other social sciences plus 2 years in direct contact with mental health consumers.  Valid driver’s license required.  Bilingual skills (Spanish/English) are a plus.

 Job Activities:

  • In a flexible and creative manner, respond to the needs of consumers and potential consumers both in an office setting (approximately 70% of the time) and in the community (approximately 30%)
  • Assess needs, and provide information and referral for behavioral health and other human services (including health insurance applications) to consumers and their families.
  • Arrange for the provision of emergency services including food, shelter, hospitalization, transportation and other basic needs.
  • Provide on-site and community-based support for new consumers awaiting assignment to agency programs including recovery coaching, and follow up for consumers discharged from these services.
  • Advocate for consumers and collaborate with other community agencies, such as DPA, social security, Mental Health Association of SE PA., other MH/MR agencies etc. in meeting the needs of the consumers.
  • Serve as the agency’s primary liaison and follow-up coordinator throughout the behavioral health commitment process
  • Assist psychiatrists, therapists, and recovery coaches in arranging for hospitalizations.
  • Assist psychiatrists in completing care plans and other related functions as needed.
  • Complete Crisis Prevention and other trainings as assigned by supervisor.
  • Complete all other duties as assigned by supervisor.

Position Overview: Provides flexible, community-based services that are designed to promote the empowerment, recovery, and community integration of adults who have serious mental health challenges by partnering with them to direct their own recovery and advocacy process, by coaching and supporting the acquisition and utilization of skills needed to facilitate the individual’s recovery, and in linking to available services and resources, including informal supports. The recovery coach will assure that services are individualized, strengths and needs based, culturally sensitive and responsive, and goal driven.

Job Duties:

  • Partnering with identified consumers to assist, encourage, and support their recovery through ongoing assessments, including recovery and prevention plans, strengths-assessments and other recovery tools
  • Collaborating with treatment team, including therapists, program coordinators, psychiatrists, wellness recovery team, peer support, and other stakeholders providing the best possible team approach to recovery
  • Linking to community and other resources and to support individuals in identifying and accessing formal and informal resources independently
  • Assisting consumers in obtaining and maintaining housing
  • Maintaining production rates to meet the program’s expectations

  • Promoting the recovery philosophy in our daily activities
