Position Qualifications:

A Recovery Coach will have one of the following:

  1. A Bachelor’s degree with major course work in sociology, social work, psychology, criminal justice, counseling.
  2. Be a registered nurse
  3. A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, criminal justice, counseling or other social sciences plus 2 years in direct contact with mental health consumers.
  4. Spanish speaking a plus.

Other Requirements:

  1. Valid driver’s license and good driving record.

Position Overview: Provides flexible, community-based services that are designed to promote the empowerment, recovery, and community integration of adults who have serious mental health challenges by partnering with them to direct their own recovery and advocacy process, by coaching and supporting the acquisition and utilization of skills needed to facilitate the individual’s recovery, and in linking to available services and resources, including informal supports. The recovery coach will assure that services are individualized, strengths and needs based, culturally sensitive and responsive, and goal driven.

Job Duties:

  1. Partnering with identified consumers to assist, encourage, and support their recovery through ongoing assessments, including recovery and prevention plans, strengths assessments and other recovery tools
  2. Collaborating with treatment team, including therapists, program coordinators, psychiatrists, wellness recovery team, peer support, and other stakeholders providing the best possible team approach to recovery
  3. Linking to community and other resources and to support individuals in identifying and accessing formal and informal resources independently
  4. Assisting consumers in obtaining and maintaining housing
  5. Maintaining production rates to meet the program’s expectations
  6. Completing documentation and other paperwork in a timely and professional manner as well as to comply with Central’s standards and state licensing requirements
  7. Promoting the recovery philosophy in our daily activities
  8. Follow CDC guidelines and agency protocols for COVID-19 in our daily work

Behavioral Health: School Based Therapist

Qualifications:

Master’s Degree in Human Services field

At least two years’ experience working with children and adolescents

Experience working in a school is preferred.

Job Requirements for the position:

Masters level clinicians to work in local school district providing individual and group sessions to identified students from grades 9-12.  Crisis counseling will be provided when necessary. This position is a full time-10 month.

Clinical staff to provide family and individual therapy to children and families in their homes.  Also will provide intensive case management services. Behavioral Health: Care Coordinator


Qualifications: Master’s Degree in social work, psychology, counseling, sociology or related field. Experience in the behavioral health field a must.

Role and Responsibilities include: Serve as a liaison between Central Behavioral Health and external agencies, including CHOC and managing the homeless clinic
? Provide walk-in case management services to high-risk and hard to engage consumers to address social determinants of health
? Serve as a care coordinator by assessing for appropriate services for consumers and facilitate those referrals and connection.
? Serve as Housing Specialist to assist consumers with housing concerns, maintain relationships with landlords and other housing resources

Behavioral Health: Nurse Navigator

Registered Nurse – Nurse Navigator

The Registered Nurse will work directly with individuals who experience serious mental illness in the role of a “Clinical Navigator.” The Clinical Navigator will work in collaboration with a Behavioral Health Professional (who holds a minimum of a Master’s degree) as part of a “Navigator Team.”

The Navigator Team (shared caseload) provides clinical services, congruent with the Recovery Model, that facilitate integration of Behavioral Health and Physical Health services.

The Clinical Navigator will complete health education, facilitate communication between Physical Health and Behavioral Health practitioners, attend appointments with individuals, participate in aftercare planning for both Psychiatric and Physical health inpatient stays, and triage complaints to the appropriate level of care. The Clinical Navigator will also use other interventions, as appropriate, to help individuals meet their wellness goals.

This position is full time, salaried working 37.5 hours per week. This is considered a community based, mobile program with the majority of the duties/tasks conducted out in the community with clients. 

Qualifications

  • The applicant must be licensed as a Registered Nurse in Pennsylvania with three years of direct care experience. Bachelor’s degree preferred but will also consider Associate Degree in Nursing.
  • Current driver’s license required.
  • Experience in both acute care and mental health preferred as the individual will be utilizing their entire body of knowledge in providing integrated care.
  • Must be able to work independently
  • Demonstrated experience working in collaborative care models.
  • Experience with health and wellness/preventive care programs.
  • Demonstrated sensitivity to and ability to serve persons of diverse backgrounds.
  • Basic computer skills a must and ability to utilize smart phones, laptops and chromebooks.

Description of Duties

  • Conduct initial assessments and enrollment into the program including a detailed history and provide education about the program and the goals and expectations of treatment.
  • Conduct, collect, and/or analyze bio psychosocial assessments of consumers, including completion of formal assessment tools and outcome measures.
  • Active participation in all outcome measurements and other QI activities within the program.
  • Serve as a consultant to consumers, family, behavioral health, primary care, and other medical offices in the community.
  • Coordinate and facilitate communication between the consumer, primary care physician and psychiatrist. Provide recommendations for changes in the treatment plan.
  • Conduct a variety of wellness and preventive care programs such as smoking cessation, nutrition, etc.
  • Monitor consumers closely (in person or by phone) for changes in severity of symptoms and medication side effects. Educate consumers about medications and their side effects, as needed; encourage treatment adherence. Screen for other medical conditions as clinically indicated.
  • Skilled in Motivational Interviewing and SBIRT.
  • Foster cooperative relationships among the diverse professional disciplines within the program and community to facilitate overall professional growth and the application of new knowledge.
  • Complete service docs thoroughly in accordance with program requirements and utilizing Collaborative Documentation.
  • Support all aspects of consumer driven recovery efforts.
  • Attend all necessary trainings and weekly/monthly meetings.
  • Participate in treatment team meetings and assist in developing integrated care plans.

DUTIES AND RESPONSIBILITIES
Position Summary: As a member of the Outpatient Services Department, the duties include engaging consumers receiving clinical services solely by a psychiatrist. This position will complete all required documentation for ongoing treatment including: treatment plans, crisis plans, and releases. The goal of the Engagement Counselor is to provide brief therapeutic interventions at least twice annually to ensure behavioral health needs are being met and make appropriate referrals to other services as needed. Conduct ASAM and clinical assessments when needed.
