Supervisor of TIP (Transition to Independence Process) Program

Staff Requirements

  1.  A master’s degree in social work, psychology, rehabilitation, activity therapies, counseling or education and 3 years mental health direct care experience
  2. Experience working with Transitional age youth
  3. Valid driver’s license and good driving record

Job Duties:

  • Facilitate flexible schedule for TIP staff which will include evening and weekend hours.
  • Oversight of TIP Program staff
  • Responsible for staff compliance with State, County, and agency policies and regulations
  • Oversight of eligibility of referrals made for TIP Program staff.
  • Coordinate county wide coverage of TIP cases
  • Work closely with the Director of the Case Management/Recovery Coach department and other Central Administration involved with TIP

Job Activities will include the following:

  • Monitor all of TIP referrals (Ages 16-26)
  • Monitoring staff charts, notes, and production
  • Complete regular performance evaluations of staff
  • Provide weekly supervision to staff
  • Hold regular staff meetings
  • Attend weekly department supervisor’s meeting and monthly full staff meeting
  • Attend required trainings
  • Ability to communicate well with families and school districts.
  • Primary point person with Montgomery County Behavioral Health department as it relates to TIP Program needs
  • Coordinate TIP’s on-call responsibilities
  • Represent Central’s TIP Program at external events as needed
  • Have the ability and knowledge to work collaboratively with County Office staff, Magellan Behavioral Health staff, Agency personnel and various outside organizations and school districts

Nurse Navigator-Behavioral Health

Registered Nurse

The Registered Nurse will work directly with individuals who experience serious mental illness in the role of a “Clinical Navigator.” The Clinical Navigator will work in collaboration with a Behavioral Health Professional (who holds a minimum of a Master’s degree) as part of a “Navigator Team.”

The Navigator Team (shared caseload) provides clinical services, congruent with the Recovery Model, that facilitate integration of Behavioral Health and Physical Health services.

The Clinical Navigator will complete health education, facilitate communication between Physical Health and Behavioral Health practitioners, attend appointments with individuals, participate in aftercare planning for both Psychiatric and Physical health inpatient stays, and triage complaints to the appropriate level of care. The Clinical Navigator will also use other interventions, as appropriate, to help individuals meet their wellness goals.

This position is full time, salaried working 37.5 hours per week. This is considered a community based, mobile program with the majority of the duties/tasks conducted out in the community with clients. 

Qualifications:

  • The applicant must be licensed as a Registered Nurse in Pennsylvania with three years of direct care experience. Bachelor’s degree preferred but will also consider Associate Degree in Nursing.
  • Current driver’s license required.
  • Experience in both acute care and mental health preferred as the individual will be utilizing their entire body of knowledge in providing integrated care.
  • Must be able to work independently
  • Demonstrated experience working in collaborative care models.
  • Experience with health and wellness/preventive care programs.
  • Demonstrated sensitivity to and ability to serve persons of diverse backgrounds.
  • Basic computer skills a must and ability to utilize smart phones, laptops and chromebooks.

Description of Duties:

  • Conduct initial assessments and enrollment into the program including a detailed history and provide education about the program and the goals and expectations of treatment.
  • Conduct, collect, and/or analyze bio psychosocial assessments of consumers, including completion of formal assessment tools and outcome measures.
  • Active participation in all outcome measurements and other QI activities within the program.
  • Serve as a consultant to consumers, family, behavioral health, primary care, and other medical offices in the community.
  • Coordinate and facilitate communication between the consumer, primary care physician and psychiatrist. Provide recommendations for changes in the treatment plan.
  • Conduct a variety of wellness and preventive care programs such as smoking cessation, nutrition, etc.
  • Monitor consumers closely (in person or by phone) for changes in severity of symptoms and medication side effects. Educate consumers about medications and their side effects, as needed; encourage treatment adherence. Screen for other medical conditions as clinically indicated.
  • Skilled in Motivational Interviewing and SBIRT.
  • Foster cooperative relationships among the diverse professional disciplines within the program and community to facilitate overall professional growth and the application of new knowledge.
  • Complete service docs thoroughly in accordance with program requirements and utilizing Collaborative Documentation.
  • Support all aspects of consumer driven recovery efforts.
  • Attend all necessary trainings and weekly/monthly meetings.
  • Participate in treatment team meetings and assist in developing integrated care plans.

Behavioral Health Navigator

Behavior Consultant/Mobile Therapist

Positions available for PT/FT employment and Fee for Service contractors.  Flexible scheduling with day and evening hours available.

Description of Duties:

The role expectations of a Behavior Consultant are as follows:

  • Ongoing design and supervision of implementation of behavior management/treatment plan for children receiving IBHS services.
  • Attendance at Clinical Planning meetings and any other mandatory meetings, observation of clinical behavior to be targeted for treatment, consultant as required.
  • Behavioral Consultant, as lead clinician, is responsible for coordinating and facilitating inter-agency team meetings, completing the treatment plan and making appropriate referrals to other supports
  • Progress notes/Documentation placed in chart on weekly basis.
  • Responsible to check in on weekly basis with IBHS Coordinator to give case updates, discuss any high-risk clients and meet with Clinical Supervisor on a monthly basis.
  • Provides behavior modification techniques to Behavioral Health Technician/Mobile Therapist to use with clients.

The role expectations of a Mobile Therapist are as follows:

  • Assess strengths and needs of child and family and include them in treatment planning process
  • Provide individual and/or family therapy, utilizing the strengths-based approach in home or other community setting.
  • Consult and collaborate with other professionals involved with case.
  • Work with Behavioral Health Technician and/or Behavior Specialist.
  • Complete all documentation required.
  • Attend any pertinent meetings as required. 
  • If lead clinician on case, Mobile Therapist is responsible for coordinating and facilitating inter-agency team meetings, completing the treatment plan and making appropriate referrals to other supports
  • If lead clinician on case, Mobile Therapist is responsible to check in on weekly basis with IBHS Coordinator to discuss any high-risk clients.
  • Meets with client for authorized number of hours per week.

Requirements:

Master’s degree, a clinical practicum and experience with children and/or adolescents required.  Ability to speak Spanish is a plus.  Willingness to travel to locations within Montgomery County.

Candidate must have a basic knowledge of computer and of Microsoft Word.

Administrative Children’s Case Manager

POSITION QUALIFICATIONS:

Bachelor’s degree with major coursework in sociology, social work, psychology, criminal justice, counseling or a related field. OR a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, criminal justice, counseling or other social sciences plus 2 years in direct contact with mental health consumers.

 Job Activities:

  • In a flexible and creative manner, respond to the needs of consumers and potential consumers both in an office setting (approximately 80% of the time) and in the community (approximately 20%)
  • Assess needs, and provide information and referral for behavioral health and other human services (including health insurance applications) to consumers and their families.
  • Assist families in accessing needed resources
  • Provide on-site and community-based support for new consumers awaiting staffing in  programs including IBHS, FBS, Outpatient therapy and non-clinical services.
  • Follow up for consumers discharged from IBHS services.
  • Advocate for consumers and collaborate with other community agencies, such as OCY, juvenile probation, MCIU, school districts, and primary care physician offices in meeting the needs of the consumers.
  • Assist Outpatient staff in arranging for hospitalizations, when needed.
  • Complete Crisis Prevention and other trainings as assigned by supervisor.
  • Complete all other duties as assigned by supervisor.

Behavioral Health Technician

Positions available for PT/FT employment

Description of Duties:

The role expectations of a Behavioral health technician are:

  • Crisis intervention techniques, behavioral reinforcements, emotional support, time-structuring activities, time-out strategies, and additional psychosocial rehabilitative activities as prescribed in the treatment plan.
  • Provide specific interventions to stabilize the child or adolescent.
  • Collaboration with other members of the treatment team and other professional working in the home or in other community settings and participation in interagency planning meetings, when possible.
  • Complete all documentation required.  Progress Notes completed within 24 hours of the service and data collection/encounters should be turned in on a weekly basis
  • Collaboration in treatment planning.

Requirements:
