Job Overview:
Central Behavioral Health is seeking a Registered Nurse (RN) to join our Food as Medicine (FAM) Program, a partnership between Central and the Pennsylvania Horticultural Society (PHS). This program integrates nutritional education, access to healthy food, and gardening opportunities to reduce health disparities for adults with serious mental illness (SMI). The RN will work closely with a Registered Dietitian, Farm Educators from PHS, and Central’s Integrated Care Team to support clients in improving their physical health and well-being.
During the non-growing season, the RN will continue to provide vital health support by participating in client support within the Clinical Wellness Program (CWRT). This program focuses on helping individuals with SMI and ongoing health issues access medical care, achieve wellness goals, and improve their overall quality of life.
Additionally, as part of Central’s Integrated Care Team, the RN will participate with:

- Other assistive departmental/project tasks as needed
Schedule:
- Full-time, Monday to Friday, 37.5 hours per week position

Responsibilities:
FAM Program:
- Collect and track vital statistics for program participants (e.g., weight, BP, BMI, lipid panel, HBA1C)
- Coordinate the collection and assessment of outcomes data for the program
- Develop and implement bi-monthly consumer groups on nutrition and health
- Create culturally appropriate educational materials for clients and community health programs
- Work alongside a Registered Dietitian and PHS Farm Educators
- Provide individual and group education on nutrition, food intake, and resource development
- Act as a health advocate, ensuring individuals receive necessary medical follow-ups
Integrated Care & Clinical Wellness:
- Participate in client support within the Clinical Wellness Program (CWRT)
- Participate with the Clozaril Clinic, labs, and other nursing tasks as needed
- Facilitate individuals’ access to medical appointments, treatment, and resources
- Serve as a healthcare resource for consumers and staff, offering workshops and trainings
- Attend weekly team meetings and agency-required meetings
Qualifications and Skills:
- Licensed as a Registered Nurse (required)
- Experience in the behavioral health field a plus.
- Strong Oral and written communication skills
- Experience with electronic charting systems preferred
