Job Overview: 

The Transitional Case Manager (TIP Facilitator) is responsible for following the evidence supported TIP Model and the Pennsylvania Blended Case Management Regulations, to support individuals experiencing emotional and/or behavioral difficulties, to achieve their independence goals.  The TIP Facilitator works to engage the young person (between the ages of 16 – 26) through relationship development, person-centered planning, and a focus on their future.  The TIP Facilitator teaches skills to help youth become more self-sufficient, connect youth to their community, build a safety net of support, advocate for youth voice and choice, and improve positive outcomes.       

Schedule: 

  • Full-time, 37.5 hours/week with flexible scheduling to support consumers’ needs. Working Monday through Friday, four days a week 11 am – 7 pm, and one day a week 9 am – 5 pm. 
  • On-call rotation with stipend reimbursement.

Responsibilities: 

  • Engage and meet with each youth on the caseload weekly.   
  • Complete forms, assessments, and case notes related to the program on time.  
  • Achieve billable hours requirements.   
  • Ability to prioritize and organize schedule.  
  • Participate in 24/7 emergency on call rotation.  
  • Available to work evening hours and weekends.  
  • Collaborate with multi-systems and family supports.  
  • Facilitate community and social events.  
  • Provide mediation and crisis support as needed.  

Qualifications and Skills: 

A TIP Facilitator will have the following: 

  1. A Bachelor’s degree with Major course work in sociology, social work, psychology, criminal justice, counseling. 
  2. Valid driver’s license and good driving record. 
  3. Spanish speaking a plus.  
  4. Ability to work both as part of a team and autonomously.  

Clearances: 

In accordance with state regulations, all prospective new employees/contractors must provide originals for a photocopy of their Federal Criminal History Record (DHS FBI Background Check), PA Criminal Background Check, and PA Child Abuse Clearance before commencing the offered position.
