🗓 Schedule:

Full-time | 37.5 hours/week with flexible scheduling to support consumers’ needs.

On-call rotation with stipend reimbursement.

🧠 About the Roles

Case Manager – Abington

The Case Manager provides flexible, community-based services designed to promote empowerment, recovery, and community integration for adults with behavioral health challenges. Using a person-centered approach, the Case Manager helps individuals and families navigate across systems, access behavioral health services, and coordinate with community and natural supports as identified in their goal plan.

Key Responsibilities:

  • Manage a caseload of no more than 30 clients in collaboration with the treatment team.
    • Partner with individuals and families to complete ongoing strengths assessments and goal planning.
    • Link individuals and families to community resources and support them in identifying and accessing formal and informal supports.
    • Complete documentation and paperwork in accordance with Central’s standards and licensing requirements.
    • Attend supervision and team meetings weekly.
    • Participate in voluntary on-call rotation with reimbursement.

TIP Facilitator – Norristown

The TIP Facilitator follows the evidence-supported Transition to Independence Process (TIP) Model and Pennsylvania Blended Case Management Regulations to support youth and young adults (ages 16–26) in achieving independence. This role focuses on relationship development, person-centered planning, and a youth-driven approach to future goals.

Key Responsibilities:

  • Engage and meet with each youth on the caseload weekly.
    • Complete forms, assessments, and case notes accurately and on time.
    • Achieve billable hours requirements.
    • Collaborate with multi-system partners, family supports, and community providers.
    • Facilitate social and community events.
    • Provide mediation and crisis support as needed.
    • Participate in 24/7 emergency on-call rotation.

🎓 Education & Experience

  • A Bachelor’s degree with major coursework in sociology, social work, psychology, criminal justice, or counseling.
  • Bilingual in Spanish/English a plus.For the Case Manager role, candidates may alternatively qualify with:
  • A high school diploma plus 12 semester credit hours in sociology, social welfare, psychology, gerontology, criminal justice, counseling, or other social sciences and 2 years of direct experience with mental health consumers.Skills
    • Valid driver’s license and good driving record.
    • Strong organizational, interpersonal, and problem-solving skills.
    • Ability to work independently and as part of a team.
    • Excellent documentation and communication skills.
