Food as Medicine

Central Behavioral Health, Pennsylvania Horticultural Society and Montgomery County Department of Health and Human Services’ Office of Managed Care Solutions have joined forces to create a Food as Medicine team consisting of a dietician, farm education manager and a registered nurse specifically designed to help adults with serious illness live better lives.

General Eligibility

If you answer yes to the questions below, you may be a candidate for our FAM program:

Are you 18 years or older?

Do you have Magellan Insurance?

Are you receiving treatment for a behavioral health issue?

Do you have a physical health condition that could benefit from improving your diet?

Are you physically able to participate in the group, including gentle movement and walking/working in the garden?

In the last 30 days, did you ever eat less than you felt you should because there wasn’t enough money for food?

Program Benefits

  • Local produce provided biweekly
  • On-site garden experience every other week
  • Learn the nutritional benefits of healthy eating/cooking
  • Improved blood pressure, weight management, and Cholesterol levels
  • Kitchen supply kits provided with enrollment

Expectations

Participants in the Food as Medicine program are expected to meet certain criteria:

  • Must meet general eligibility guidelines
  • Must be willing to participate in group programming twice a month
  • Complete nutritional screening
  • Complete surveys and participate in periodic health monitoring like BP and weight (Gift cards provided with completion)
  • Able to participate in the program through mid-November
