At Central, we know that good physical health care and mental health care go hand in hand for optimal health. For people struggling with behavioral health issues, it can seem overwhelming to deal with a chronic underlying medical condition at the same time. With your Clinical Wellness Recovery Team by your side, you can count on coordinated care that best supports your overall physical and mental health.

How it works

Your team consists of a registered nurse navigator and a behavioral health navigator, who together will serve as your advocates to ensure you receive ongoing, appropriate care. They will work with your primary care provider and other health care professionals — including your psychiatrist, treatment team members and any other medical or behavioral health care providers you see — to achieve better coordinated care. They’ll also communicate with anyone who’s part of your personal support system.

Depending on your situation, your team can:

  • Help you gain access to health services and screenings
  • Share educational information about medical conditions, medications, quitting tobacco, exercise, nutrition and important health tips
  • Support you in the community, at medical appointments and during evaluations
  • Advocate for and arrange medical follow-up care
  • Provide health monitoring
  • Guide you if you’re having trouble following recommended treatment

Clinical Wellness Recovery Team services eligibility

To be eligible to receive these services, you must:

Be age 18 or older

Live in eastern or central Montgomery County

Have been diagnosed with a serious mental illness as well as a chronic physical health problem

Have Magellan behavioral health coverage

Submit a psychiatric evaluation that was conducted within the past year
