Case Management services for children, teens and young adults
If your child has received a mental health diagnosis, you probably have lots of questions and concerns. You might be wondering what to do next, how to advocate for your child, or where to turn for help when your family is struggling.
Our Case Management services for children, teens, and young adults are designed to:
- Reduce behavioral health hospitalizations
- Avoid out-of-home placements for children and teens with emotional, behavioral, and/or substance abuse problems
- Promote Socializations/Peer support
- Coordinate appropriate mental health services
- Help family members communicate more effectively and support treatment
A child, teen, or young adult enrolled in our “Blended Case Management” service is assigned to a case manager. There is no cost to you as this service is provided thanks to federal, state, and county funding.
We partner with your family to coordinate
- Family and community support
- Assistance with income support and entitlements
- Mental health treatment
- Socialization and peer support
- Advocacy, information and referrals
- Crisis intervention
- Help with physical health care needs
- Assistance with housing needs
Case Management Services Eligibility
To be eligible for these services, your child should be:
A Montgomery County resident under 21 years old
Living in Central Montgomery County (Norristown, Conshohocken, Bridgeport, Lafayette Hill, King of Prussia or a nearby community)
Still enrolled in school
Undergoing treatment for emotional and/or behavioral problems
Referrals
You can refer a child, teen, or young adult to our Case Management services by completing our referral application.
