Our Senior Intensive Support (SIS) services, formerly called “Senior Care Case Management,” are designed to support seniors who struggle to care for their daily needs, allowing them to remain in their homes and thrive independently in the community.

SIS services are funded by and offered in collaboration with the Montgomery County Department of Health and Human Services’ Office of Senior Services.

Our SIS team can help

  • Provide emotional support and resource coordination
  • Increase socialization opportunities and build relationships
  • Linkage with community supports
  • Transportation Coordination
  • Provide medication monitoring
  • Environmental management and techniques for performing activities of daily living
  • Family education and support
  • Strategies for budgeting and money management
  • Health and wellness goals

SIS Services Eligibility

To be eligible for these services, participants must:

Be age 60 or older

Live in Montgomery County

Need support with caring for their daily needs, whether that’s obtaining food, managing medications, completing daily activities, managing finances or finding connections to achieve an active, rewarding life
