🧠 About the SIS Case Manager Role
The Case Manager for Seniors provides flexible, community-based services that are designed to promote empowerment, recovery, and community integration of older adults (ages 60+) with behavioral health challenges. Using a person-centered approach, the case manager helps the individual navigate across systems, gain access to behavioral health services, and coordinate with community and natural supports as identified on their goal plan.

📝 Responsibilities
- Support older persons to remain in their own homes and to live independently for as long as possible
- Connect older persons to needed resources (food, shelter, transportation etc…) and natural supports.
- Outreach, assessment, advocacy and follow-up.
- Support older persons in connecting to needed clinical and medical services.
- Collaborate with Central’s internal team, Montgomery County Office of Senior Services and other community services.
- Complete all necessary documentation
🎓 Qualifications
Education & Experience
- High School diploma or equivalent
Skills
- Valid driver’s license and good driving record.
- Willingness to work with older adults.
- Ability to work both as part of a team and autonomously.
- Excellent attention to detail and organizational skills
- Customer Service skills
- Ability to manage crisis and problem solve
