Case Manager-Senior Services

Job Overview:

The Case Manager for Seniors provides flexible, community-based services that are designed to promote the empowerment, recovery, and community integration of older adults (ages 60+) with behavioral health challenges. Using a person-centered approach, the case manager helps the individual navigate across systems, gain access to behavioral health services, and coordinate with community and natural supports as identified on their goal plan.

Schedule:

  • Full-time, 37.5 hours/week with flexible scheduling to support consumers’ needs.

Responsibilities:

  • Support older persons to remain in their own homes and to live independently for as long as possible
  • Connect older persons to needed resources (food, shelter, transportation etc…) and natural supports.
  • Outreach, assessment, advocacy and follow up.
  • Support older persons in connecting to needed clinical and medical services.
  • Collaborate with Central’s internal team, Montgomery County Office of Senior Services and other community services.
  • Complete all necessary documentation

Qualifications and Skills:

  • High School diploma or equivalent
  • Willingness to work with older adults.
  • Valid driver’s license and good driving record.

Clearances

In accordance with state regulations, all prospective new employees/contractors must provide originals for a photocopy of their Federal Criminal History Record (DHS FBI Background Check), PA Criminal Background Check, and PA Child Abuse Clearance before commencing the offered position.
