Registered Dietician  

Job Overview:

Starting in May 2024, Central will kick off the Food as Medicine (FAM for short) program. FAM is a partnership between Central and the Pennsylvania Horticultural Society (PHS).The program leverages this partnership to promote a Food as Medicine model and provide staff and clients at Central with nutritional education and resources to address and reduce health disparities. Through this program, individuals served at Central are able to access healthy food and gardening opportunities and learn nutritional and culinary skills.

The Registered Dietician (RD) provides nutritional assessments, trainings, and group demonstrations around healthy eating. The dietician works with adults with SMI around issues of nutrition, food intake, and resource development. The RD works closely with a Registered Nurse, and Farm Educators from the PA Horticultural Society.

Responsibilities:

  • Provide nutrition assessments and care plans for consumers in the FAM program
  • Work alongside PHS team to develop and deliver prescription based “Food as Medicine” boxes
  • Participate in the development and implementation of bimonthly consumer groups, providing food demonstrations and fresh produce from the on-site garden
  • Develop culturally appropriate educational materials on nutritional issues for use with individual consumers and community health education programs.
  • Documents all consumer care in the electronic health record.
  • Serve as an expert resource to all consumers and staff members, providing workshops and trainings as needed.
  • Assist in the collection of data for the FAM program
  • Attend weekly team meetings and other meetings as required.

Qualifications and Skills:

  • Certified as a Registered Dietician (required)
  • Bachelor’s degree in Nutrition or related field
  • Strong Oral and written communication skills
  • Experience with electronic charting systems preferred

Responsibilities:

  • In a flexible and creative manner, respond to the needs of consumers and potential consumers both in an office setting (approximately 70% of the time) and in the community (approximately 30%).
  • Provide on-site and community-based support for new consumers awaiting assignment to agency programs including recovery coaching, and follow up for consumers discharged from these services.
  • Serve as the agency’s primary liaison and follow-up coordinator throughout the behavioral health commitment process
  • Assess needs, and provide information and referral for behavioral health and other human services (including health insurance applications) to consumers and their families.

Responsibilities:

  • Conduct initial assessments and enrollment into the program including a detailed history and provide education about the program and the goals and expectations of treatment.
  • Conduct, collect, and/or analyze bio psychosocial assessments of consumers, including completion of formal assessment tools and outcome measures.

  • Monitor consumers closely (in person or by phone) for changes in severity of symptoms and medication side effects. Educate consumers about medications and their side effects, as needed; encourage treatment adherence. Screen for other medical conditions as clinically indicated.
  • Skilled in Motivational Interviewing and SBIRT.
  • Foster cooperative relationships among the diverse professional disciplines within the program and community to facilitate overall professional growth and the application of new knowledge.
  • Complete service docs thoroughly in the DAP format.
  • Support and actively engage in consumer-driven recovery efforts, contributing to treatment team meetings and assisting in developing integrated care plans.
  • Attend all necessary trainings and weekly/monthly meetings.

Qualifications and Skills:

  • Master’s degree in a relevant field is required.
  • Preferred experience in mental health, demonstrating comprehensive knowledge for providing integrated care.
  • Must be able to work independently and efficiently.
  • Demonstrated experience working in collaborative care models, with experience in health and wellness/preventive care programs.
  • Demonstrated sensitivity and capability in serving individuals from diverse backgrounds.

Responsibilities: 

  • Design & Supervision: Develop and oversee behavior management/treatment plans for children in IBHS services.
  • Collaborative Meetings: Attend and contribute to clinical planning meetings, and other mandatory meetings offering expertise and guidance as required.

Position Qualifications and Skills: 

  1. Be licensed in this Commonwealth as a behavior specialist.
  2. Have obtained an MA or MS in psychology, social work, education, or counseling with 1 year of Full-time experience in providing mental health direct services to children, youth, or young adults.
  3. Have obtained an MS or MA, in psychology, social work, education, counseling, and completed a clinical or mental health direct service practicum.
  4. Certification as a BCBA or graduate degree in Behavior Analysis.
